Please provide the amount your parish/school, School Choice, or some other entity associated with your parish school actually provided to families during the 2015-2016 school year. DO NOT include tuition assistance from the Catholic Education Foundation. Please complete form by Friday, July 8, 2016.
  • Please provide the actual tuition assistance granted from your parish/school to families for the 2015-2016 school year. Do NOT include funds provided by the CEF. If none, please put N/A.
  • Please provide any tuition assistance granted to families for the 2015-2016 from School Choice.
  • Please provide any tuition assistance granted to families for the 2015-2016 school year from other sources (NOT including CEF). This could be another parish, a separate endowment of the parish, etc. Please also provide the source of the funds.
