H1N1 FLU UPDATE
School Vaccine Clinics

Archdiocese of Louisville Catholic schools in Jefferson County are working with the Louisville Metro Department of Public Health and Wellness to offer students free H1N1 flu vaccinations. Each school has been assigned a date, and your school will communicate to you the date the vaccines will be offered at your school. The attached link is a listing of the dates the vaccine will be offered at each school.

All schools will send parents an information packet with a permission form. To be sure their children receive a vaccination, parents will need to fill out the form completely and return it to the school as soon as possible. The permission form also will be available on the health department web site (www.louisvilleky.gov/health).

The Centers for Disease Control and Prevention Advisory Committee on Immunization Practices has recommended that children and young adults ages 6 months through 24 years receive the H1N1 vaccine as soon as possible.

Parents who have concerns about their child’s eligibility for the vaccination should contact their health care provider before signing the permission form, especially if the child has an underlying health condition. For other medical or health-related questions, please go to your child’s medical provider, the LMDPHW H1N1 hotline (568-4161), the Kentucky Health Department hotline (1-877-843-7727), www.flu.gov orwww.cdc.gov. School and archdiocesan personnel cannot answer any medical questions regarding the H1N1 vaccine.
